Author's choice, and the older themes were focused mainly on system icons. The more advanced functions in IP v3 have set a new standard for including file type icons. The newer icon packages include a ".doc" file type icon, and artists are incouraged to include a greater number of file type icons.

This is an issue that has been going on for a while, and may be due to the default program for ".doc" files is WordPad - which is included with the Windows XP operating system.
It would seem that the "Restore all Icons" function is looking at the OS defaults as a primary, and then the installed programs if the OS does not have an association available for the file type.
I am not a coder, so do not know how one writes the "Restore" function coding, but based on behavior this seems to be the logical answer.
The current work around is to use the "Folder Options > File Types tab > select ".DOC" file > advanced button > assign the icon from the "WINWORD.EXE" file.
Hopefully this one will be fixed in future "official release" versions.
