What's with old icons coming back?

Hey all,

I have a problem with Icon Packager v3.0 - If i apply an icon package that doesn't have an icon, such as a .TXT file icon, it will replace it with a .TXT icon from a different pack! The program just randomly replaces un-skinned icons with one from a random different icon pack. How can i stop this from happening?
