Well, I had to grab your attention somehow... So here's my suggestion:

CTRL+CLICK to launch a new instance of a program

Obviously applies only for those of us using the "When a shortcut is ran, if the program is already running bring it to the foreground only".

This would be great for me: launching new instances of Visual Studio or bringing up the existing! I can't use the "Display open windows along with my icons" as I always have way too many apps & windows opened.

Mmmnn... this should be combined with a pop-up above the icon on mouse-over listing the window titles of the different instances, if applicable.
